Advancing Teams in Community Health
Charles River Community Health received a grant to use patient navigators/community health workers to improve care for patients with complex care needs. Funding was made available to advance care coordination, enhance patient outcomes, and increase provider, staff, and trainee satisfaction. Projects are to help catalyze the transformation to high functioning, team-based primary care practices serving the most vulnerable patients and communities. Grantees attended a conference at Harvard Medical School yesterday to report on their progress and continue to work toward achieving their goals.
