We’re pleased to receive recognition from the Health Resources and Services Administration (HRSA) in several key areas in 2024. In addition to recertification of our Patient Centered Medical Home status at both our Brighton and Waltham Sites, we also earned two badges our work!
Advancing Health Information Technology (HIT) for Quality which is awarded to health centers who meet all the following criteria:
- Adopted an electronic health record (EHR) system.
- Offered telehealth services.
- Exchanged clinical information online with key providers’ health care settings.
- Engaged patients through health IT.
- Collected data on patient social risk factors
Addressing Social Risk Factors which is awarded to health centers that:
Increase the percentage of patients who received enabling services between the last two UDS reporting years and collect data on patient social risk factors.
