CRCH Receives Grant to Support Patients Living with Chronic Conditions

Charles River Community Health was recently awarded a grant of $23,998 from The Rabbi Abraham Halbfinger and Charlesview Charitable Fund. The grant will be used to support patients living with chronic conditions such as diabetes, hypertension and chronic pain. CRCH will offer the Stanford Chronic Disease Self-Management Program (CDSMP), a six-week program that assists with problem-solving, decision making and education related to managing chronic conditions. Additionally, CRCH will offer group medical visits to continue supporting and educating patients.  The Rabbi Abraham Halbfinger and Charlesview Charitable Fund aims to help the Allston-Brighton community’s efforts to be a thriving neighborhood where opportunities abound, and ALL residents can reach their potential.
