Charles River Community Health was recently recognized by the American Heart Association for our efforts in cholesterol management, hypertension management, and Type 2 diabetes management based on our 2022 outcomes.

Check. Change. Control. Cholesterol

This initiative aims to improve awareness, detection, and management of high cholesterol by educating and empowering consumers, healthcare professionals, and patients with evidence-based information and tools, while also improving quality of care for patients through American Heart Association’s quality improvement programs.

Target:  Type 2 Diabetes

This program is a quality improvement and recognition opportunity for organizations providing outpatient care related to type 2 diabetes and cardiovascular risk factors. Sponsored by the American Heart Association and Know Diabetes by Heart™, the initiative brings together resources, education materials, regional AHA staff support, and recognition opportunities to target improvement and acknowledge organizations for their impactful work.

Target BP

The Blood Pressure Recognition Program celebrates physician practices and health systems, who treat patients with hypertension, for achieving blood pressure control rates at or above 70 percent within the populations they serve. These achievements should ultimately lead to a reduction in the number of Americans who suffer heart attacks and strokes. Silver recognition indicates that we submitted our data and attested to providing 4-6 evidence-based activities. Activities include categories in equipment calibration and validation, staff knowledge and skills, and system of care.

A multidisciplinary effort at CRCH helped earn this important recognition. Staff from of all the medical and clinical teams were involved.  Population Health identified patients who were eligible to participate and made calls to encourage them to come in for a screening.  The front desk staff checked-in patients, the nursing staff checked blood pressure and administered point-of-care tests and the pharmacy filled prescriptions and explained how the medication worked.  Congratulations to everyone involved in helping to care for our community!
